Travel For Minors


Children under the age of 18 traveling out of the country without a legal guardian must carry with them a notarized parental consent form. Please review the appropriate forms and have them completed prior to travel. These must be current with a recent notarization seal. They may be checked by airline personal upon departure from the U.S.

Additionally, if a minor under the age of 18 is traveling out of the country with only one legal guardian, the other must give consent via notarized form. If there is no second legal guardian, the guardian with sole custody must also complete a notarized form. These must be current with a recent notarization seal

Forms for one legal guardian to accompany child out of the country

Other important items for minors traveling out of the country without a legal guardian:

  • If there is another designated adult traveling with the minor that can make emergency medical decisions for the child, please create and notarize a letter that reflects temporary medial power of attorney.
  • Have the child carry their own medical insurance information and relevant health information, and/or provide a copy to the individual that has been designated to have temporary medical power of attorney.
